I have made raised-letter nameplates a couple ways. The first, and simplest, is to cut the letters out of sheet plastic and glue them to the nameplate. A nice sharp knife blade and some needle files are a great help.
The second is a little slicker, although it costs a bit. Send your art to a rubber stamp company (I usually use rubberstamps.com) and have a rubber stamp made of your nameplate, then copy it in resin using a rubber mold and two-part polyurethane resin. You have to order a stamp that prints a reversed image.

I traced these letters that I copied from the kit instructions onto a flattened piece of Super Sculpey, cut them out and baked them to harden. I cut a piece of basswood which was then squared up, sanded and beveled. I glued the letters to the basswood and then sanded the letters smooth and to a uniform thickness. The assembly was painted black and the letters picked out with metallic wax (similar to Rub n Buff).
